New Patients

If you are ready to schedule your first appointment, please call, email, or chat with us. 

Our administrative staff is ready to take your call Monday – Friday 8:00 a.m. – 5:00 p.m. and can answer all of your questions about our doctors and our practice.

After you schedule your first appointment, you will receive a welcome email with a new patient packet. We encourage you to read through the packet which provides helpful information to get the most out of your first appointment at CPCH. Please fill out any required information in your new patient packet and return this information to our admin staff in order to receive your appointment date and time.

Your doctor will be waiting to see you at your scheduled appointment time so if you arrive late for your appointment it decreases the amount of time you will have to speak with your doctor. Please plan to arrive 30 minutes before your first appointment so you don’t waste any of your time with your doctor!

On February 27rd, our current electronic health system will transition to a new and advanced system to better serve you: Athena. Prior to the transition date, you will be sent a registration link to create a new patient account in Athena. If you have any immediate questions or concerns, please do not hesitate to contact your therapist, or call our office to speak to a staff member.
